One such form is ADOPT – 330, which is required to request for appointment of a confidential intermediary. This form states that you want to seek contact of your sibling who is not a dependent of the court or one of you who has been adopted. However, before completing this form, you must ask for contact details of your sibling from the department or licensed agency that helped in the adoption or your sibling’s adoption.
After filling up the form, give it and a blank copy of the proposed legal order to the clerk in the court where the adoption was finalized. In addition to all, after the court checks all the legal documents submitted by you in the court and sign the order for you. Therefore, you can collect a copy of this request.
